The consultant psychiatrist, the patient’s focal therapist and nursing team liaise with GPs and referring agencies, to make sure that they are updated and involved in this process and that arrangements are in place to support patients’ ongoing care.
Our specialist team devise a personalised discharge plan with the patient which may include recommendations for further individual and group counselling and emphasises the importance of attending fellowship meetings – such as Alcoholics Anonymous (AA) / Narcotics Anonymous (NA) regularly.
